Provision Care LLC (“Provision,” “we,” “us,” or “our”) operates the website provisioncare.ai and the elder care assessment tools available through it (collectively, the “Service”). This Privacy Policy explains how we collect, use, disclose, and protect your information when you use the Service.
By using the Service, you agree to the collection and use of information in accordance with this Privacy Policy.
1. Information We Collect
Information You Provide Directly
When you use our care assessment, you provide information about your parent or loved one, including their health status, living situation, cognitive status, and care needs (“Assessment Data”). You may also provide your own name and email address if you choose to receive your care report or other communications from us.
Please note that Assessment Data may include information about a third party (your parent or loved one). By submitting Assessment Data, you represent that you have the authority to provide this information and that doing so does not violate any obligation you owe to the individual whose information you provide.
Information Collected Automatically
When you visit provisioncare.ai, we automatically collect certain technical information, including:
- IP address
- Browser type and version
- Device type and operating system
- Pages visited, time spent on pages, and referring URL
- Cookies and similar tracking technologies

- Health-related Assessment Data is used solely to generate your care report and to improve the Service in anonymized or aggregated form.
- We do not share health-related Assessment Data with third parties for their own purposes.
- Under the California Consumer Privacy Act (as amended by the CPRA), health information is classified as “sensitive personal information.” We limit our use and disclosure of sensitive personal information to what is necessary to provide the Service.
- Under the GDPR, health data is a special category of personal data. Our legal basis for processing health-related Assessment Data is your explicit consent, provided when you voluntarily submit the assessment.
3. How We Share Your Information
We may share your information only in the following circumstances:
- Service Providers: With third-party vendors who perform services on our behalf (such as hosting, analytics, and email delivery), subject to confidentiality obligations.
- Legal Requirements: If required by law, regulation, legal process, or governmental request.

7. CAN-SPAM Compliance
If you provide your email address, we will:
- Not use false or misleading subjects or email addresses
- Identify any commercial message as an advertisement where required

- Allow you to unsubscribe from any email communication at any time via the unsubscribe link included in each email
8. Do Not Track
Some browsers offer a “Do Not Track” (DNT) setting. There is currently no industry standard for how companies should respond to DNT signals. At this time, we do not respond to DNT signals. If a standard is adopted in the future, we will update this Privacy Policy accordingly.
9. Children’s Privacy
